Tying a pedicle to be cut in between ligatures is done by using the ties on passers, & can be done by completing the following steps.
Step 1. The surgeon places an angled clamp beneath the pedicle and opens the jaws when it emerges on the other side of the tissue. Now the surgeon's nurse and/or assistant will place a tie on a passer, held taut between the free end in one hand and the clamp in the other hand, deep to the angled clamp's jaws. Using the angled clamps the surgeon will then grab the taut portion. By grabing a taut segment, expect the free side hanging under-neath the passer, the operators hold control of the ligature.
Step 2. The doctor will then take a short moment to pause until he sees that the ligature has been released, before pulling it through. Clamping and pulling the ligature in one motion before it has been let go then becomes a torn pedicle that gives both embarrassing to operators & hazardous to the patient.
Step 3. The doctor grabs the free ends in one hand, leaving the assistant with both hands free to pass the 2nd ligature on to the reinserted angled clamp's jaws.
Step 4. The nurse ties their sides 1st and then retains the second ligature. Slight retraction in the correct direction of the single ligature will help keep distance between the two while the other is tied. Tying of 1 ligature can be countertraction may sometimes be aided when grabing the other end of the ligature deep inside the wound with a surgical instrument and appropriately directing its tension.
To assure adequate distance between the two ties, the knot tier forms the 1st half hitch, then prior to tying it down, inside the loop you will want to place your two index fingers. He then positions the deep portion of the loop against the deep side of the pedicle at the appropriate place, then this completes the tie. with a passer as in a vessel ligation, You want to make sure before you tie the first hitch that you facilitate the segments, with best visibility for both assistant and tier.
